Overview

Assists eligible recipients to gain access to needed medical, behavioral, health, social, educational, and other services. Establish processes to coordinate a range of services for instruction and assistance for persons served and their families. Assists in supervising Case Managers.

External Job Description
    Serves as backup support to the case management supervisor in their absence
  • Oversees timeliness and quality of all case management functions
  • Supervises and oversees the case management process the provision of services included in the service plan the reassessment process and case management documentation process
  • Manages the completion of detailed review of advantage member client chart slash files for proper reutilization of resources
  • Adheres to requirements for documentation and reimbursement consistent with regulatory compliance and corporate policies procedures and standards
  • Recommends courses of action to case managers and advise them in the proper course of action and care
  • Ensures that all case management staff were oriented to company policy and procedures and advantage case management policies and procedures including form usage upon hire and ongoing participates in the initial orientation of the staff regarding advantage requirements for care and documentation also training as necessary for new hires and retraining on previous hires to include advantage training policy and procedures billing policies and out in the field training
  • Views documentation of the service plan to ensure consistency with the advantage program philosophy and capacity to serve
  • Reviews documentation to ensure that service plan goals objectives action steps and expected outcomes correspond to the consumer's challenges and strengths identified in the Ucat the case management referral the IDT and the monitoring of reassessment findings if applicable
  • Reviews reports and spreadsheets to help assist CMS with keeping up with all monthly monitors five-day follow-ups reassessments in other case management job duties are completed in a timely manner and turned in on time to advantage standards
  • Holds small caseload and helps with overflow
Qualifications
  • Bachelor's degree in a Human Services field or Nursing field
  • Two years of experience with case management or related discipline, prior supervisory experience preferred
  • Active/valid Nursing license from the state in which you are performing services
  • Ability to communicate both verbally and in writing
  • Minimal travel may be required

USD $26.00 - $28.00 / Hour
